MEMBERSHIP PLANS AND MERCHANDISE

                    I, the undersigned, do hereby voluntarily submit my application for the enrollment and participation in martial art training and events sponsored by OASIS JUI-JITSU LLC (“OASIS”) I understand that participation in a martial art is a highly physical activity that can result in serious injury and even death and I hereby assume that risk. I hereby release OASIS, and all of it’s officers, members, agents, instructors, owners and authorized guests, individually or otherwise, including but not limited to Jim Hughes, Royce Gracie and the Royce Gracie Jiu-Jitsu Network, and any and all Landlords for any and all liabilities and/or claims of injury, accident, death and or losses I may sustain or incur, physically, financially, mentally, or emotionally, if any, while attending classes, training sessions, seminars or events, and I hereby agree to indemnify and hold each of them harmless in the event of personal injury from the accidental, intentional, malicious, normal or abnormal use of the techniques applied before, during, and after the classes, training sessions, seminars or events. This indemnification includes but is not limited to any attorney’s fees and other costs incurred by any of the indemnities in defending any action brought by the undersigned against them by me or my representatives as the result of injuries sustained due to my participation.

I, the undersigned, hereby agree to individually provide for the possible future medical expenses which may be incurred as a result of any injury sustained while participating in any such training session, seminar or event. I hereby give my permission to medical professionals to administer emergency medical aid should sickness, injury, or accident occur. I agree to allow Oasis and Royce Gracie Network to freely use my image and likeness in media, television production, and promotional materials.

I agree to abide by all rules and to train safely. I agree to wear proper safety equipment such as a protective mouthpiece. I agree to maintain good personal hygiene and wear a clean approved uniform and to not wear the uniform or patches of competitors of Royce Gracie Jiu Jitsu.

I understand that Royce Gracie Jiu-JitsuÔ is a registered trademark and cannot be used without express written permission. I agree that I will not compete with Oasis and I will not recruit Oasis students for other martial arts schools or clubs or take any action which will dilute the membership of the Oasis and its network of schools without written consent.

I agree to contribute $30 yearly association dues when I start and again each January of my membership. I agree to contribute my dues according to the membership I choose below.

I agree that my monthly dues and yearly association dues may be automatically deducted from my checking account as a RECURRING CHARGE and I will provide a voided check to initiate the bank transfer. I am making a minimum one year commitment to support the organization in its efforts to support me. My membership will continue on a month to month basis upon the completion of one year of membership. WHEN I DECIDE TO TERMINATE MY MEMBERSHIP AFTER MY INITIAL ONE YEAR COMMITMENT, I WILL SEND A REGISTERED LETTER TO JIM HUGHES AT: 41 CROSSROADS PLAZA #300, WEST HARTFORD CT 06117 STATING MY INTENTIONS AND THE AUTOMATIC TRANSFER FROM CHECKING WILL STOP ON THE NEXT PAYMENT CYCLE. I understand that written instruction by registered mail is the only method for assuring absolute clarity of my intentions and therefore the only means of canceling membership.  I understand that contributions, once made, are not refundable. Adult members may train at all locations unlimited. The Private Training Plan includes 3 private or semi-private lessons per month and unlimited group classes.

Only prospective members who are willing to participate in the monthly EFT program will be allowed to join.  I have read the above, I understand the meaning of this information, I sign voluntarily to its content and intent, and I have received a copy of this agreement. I agree to allow a background check to verify my criminal history. This agreement and the releases herein are binding on the undersigned, and his or her heirs, successors, administrators, executors and assigns.
